Theory underpinning nursing practice
One theory that underpins nursing practice is the Person-Centred Care (PCC) model. This theory emphasizes that the patient’s unique perspective, values, and beliefs should be the focus of the care provided by nurses (McCormack & McCance, 2017). The PCC model aligns with my nursing practice as it provides a framework for building a therapeutic relationship with patients, which is essential in delivering high-quality care.

The PCC model emphasizes the need for nurses to understand the patient’s perspectives, which requires a deep understanding of the individual’s unique context, cultural background, and lived experiences. Nurses must also be able to effectively communicate and engage with patients in a way that acknowledges and respects their values and preferences. This is particularly important when managing chronic illnesses or complex health problems, as patients often face multiple challenges in their daily lives that affect their health outcomes (McCormack & McCance, 2017).

By using the PCC model, nurses can develop care plans that are tailored to the individual patient’s needs, which can result in better health outcomes, increased patient satisfaction, and improved quality of life. For example, in my clinical practice, I have worked with patients who have chronic illnesses, such as diabetes, who have complex health needs that require ongoing support and management. By using the PCC model, I have been able to develop care plans that address the patients’ unique needs, which has resulted in improved health outcomes and better quality of life for my patients.
